Prismatic Exchange Donation Form

Donation Options

You can choose multiple options and we will do our best to split the donation up for each choice.

Option 1: Christmas Donation

The Christmas Angels donation is a local donation in Arizona that provides assistance for foster families and foster kids in need that can't afford gifts during Christmas time. I'll hold the cards or product you send and create packs and goodie bags like what's listed on YouTube and TikTok, then my wife and I will send out the packs/product in December when the program opens up again.

Option 2: Order Donation

Order donation goes straight to kids/teens/young adults that purchase product at Prismatic Exchange. When an order comes in for younger patrons we try and throw in extra product to make their order special.

Option 3: Content Donation "Giveaway"

This last option helps the channel grow and reach the right audience in the form of a giveaway. The donation you send will be recorded and uploaded in the form of a video to TikTok and YouTube with shout out to your handle.

Enter in any social media handle you'd like to be tagged with and the platform(s) of choice.

Shipping

At the moment we are offering reimbursement for shipping to us for donations! In the future this may change however we're happy to be able to offer reimbursement when sending donations to Prismatic Exchange at this time.

How it works: Once you upload an image of your receipt for shipping we'll reimburse up to $15 once we receive the package.

This will be done through Pay Pal Goods and Services or store credit, after the product has been received.

I acknowledge that the products I am providing are intended as a donation and will be treated accordingly. I confirm that I am the sole owner of these cards and willingly transfer ownership to Prismatic Exchange for donation purposes.

I acknowledge that I am at least 18 years of age or older. If under 18, I confirm that I have explicit permission from my parent or legal guardian, who is facilitating the shipment of these items on my behalf. By sending these products, I affirm that I (or my guardian) fully consent to transferring ownership to Prismatic Exchange and understand the terms and conditions of this donation.

I acknowledge that I will be reimbursed for my shipping costs, up to a maximum of $15 USD, either via PayPal Goods and Services or as store credit, upon confirmation that Prismatic Exchange has received the donated products. I also understand that it is my responsibility to obtain a tracking number for the shipment and to provide this tracking information to Prismatic Exchange. Failure to provide tracking details may result in delays or denial of reimbursement.

I acknowledge that by providing my personal information—name, email address, and date of birth—I consent to Prismatic Exchange securely storing and using this data solely for the purpose of verifying and managing my donation. If I am under 18, I confirm this is done with the permission of a parent or legal guardian. Prismatic Exchange will not share or sell this information.

I personally wanted to thank you for taking the time to fill out this form and your consideration to donate cards and tcg products for those less fortunate. To everyone that donates I respect every decision you make for donating anything to Prismatic Exchange as it helps deliver more cards to the community and to those who can't normally get product.
